2 most recent paystubs (covering recent 30 days) and /or most recent Benefit letters (any fixed income, retirement, Social Security, etc.)
Last paystub issued 12/2023 & 12/2024 (if you have access to it, if not, Please send name, phone, and email (1 of which must register to employer) for someone who can complete a verification of Employment for each employer you worked with from 2023-YTD)
Copy of Driver’s License (s) & Social Security Card (s) OR legal documents showing full SS#(s) and name(s)
